Requests to block adverse information resulting from human trafficking from a registrant’s consumer report may be made via telephone, email, or mail, at:

To block adverse information resulting from human trafficking, the request must be submitted with the following information:

  • Proof of identity via government-issued identification or other means, including a driver’s license, passport, utility bills, or other government identification. Representatives assisting with the submission should also provide proof of identity.
  • Identification of the adverse information that resulted from trafficking, using the Trafficking Victim’s Template or other document which includes the following information:
    • Items from trafficking that should be blocked from your report, by the name of the source (e.g., State Court)
    • Type of Item (e.g., Criminal Conviction)
  • Trafficking victim determination in any of the following forms of documentation:
    • Court documents from a federal, state, or tribal government entity;
    • Documentation from a non-governmental organization or member of a human trafficking task force authorized by the government to make such a determination;
    • Court documents showing victim status; or
    • A self-attestation, signed by you or your representative attesting that you are a victim of trafficking, if it is signed by a representative from a government entity, an authorized non-governmental organization, a human trafficking task force, or a court.
